*All searches above will be conducted in all jurisdictions where the applicant has lived and/or worked (company headquarters and physical location) within the past 7 years and will include all provided and developed names. A Minnesota statewide will also be conducted through the Minnesota Court Records Online (MCRO) on all applicants. A New York statewide through the Office of Court Administration (OCA) will also be required on all applicants who have lived and/or worked in the state for the within the past 7 years.

- Please provide all required information and electronically sign forms.
- Complete the one-time payment process fee of $295.00 via Credit Card.
- Upon completion of the background screening process, CSI will provide an authorized user at MN OCM with a copy of the background screening report.
