This section provides clients with the forms needed in the background screening process. Standard Forms must be signed by the applicants and provided to CSI accompanied by the background screening request before an investigation can be initiated. The FCRA Forms are samples of the letters that are issued to applicants when the decision not to hire has been made based on the adverse information in the background screening report provided by CSI.
